Catholic Health is opening a new Ambulatory & Urgent Care location in Centereach. The multispecialty practice is located at 2112 Middle Country Road and is set to open in July 2023. This new multispecialty location will offer an array of primary and specialty care, as well as urgent care services for the Centereach community and surrounding areas.

Position Summary:

Coordinates a patient-focused team in providing direct and collaborative health care in a variety of clinical settings designed to promote and restore optimal health across the life span. Utilizes all components of the nursing process in providing care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights. Utilizes knowledge and expertise in case management to provide comprehensive, coordinated, seamless care to individuals and aggregate groups. Coordinates, integrates and facilitates the patient/provider relationship along the continuum of care.

Responsibilities: 

• Plans to meet needs of each patient based on the collection of subjective & objective data obtained from patient, family, & other staff members.
• Ensures proper completion of admission process which includes: identification of patient; taking vital signs; assuring completeness & accuracy of operative consent forms & other required laboratory reports.
• Report to Gastroenterologist/Anesthesiologist any pertinent observations noted during admission process regarding symptoms or reactions.
• Assists physician during procedure as directed by the physician.
• Performs post procedure call backs. Refers patient to the appropriate resource if any needs are identified.
• Demonstrates the ability to constructively evaluate & communicate to staff members on issues concerning patient care.
• Is accurate, concise, & relevant in reporting/recording all data required for patient, nursing service, and hospital.
• Uses nursing charting forms appropriately.
• Uses appropriate nursing charting format.

Qualifications:

• Graduate of an accredited school of nursing. Bachelors Degree preferred. RN certification required or must be working toward certification.
• Licensed Registered Nurse. BLS and ACLS certification required.
• Minimum of two years practice in a medical/surgical setting required and one year experience in ambulatory surgery / Endoscopy practice.

The range for this position is $39.08 – $63.21 per hour.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies, and experience and position location. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
